Grade 7 School Immunization Clinic

This is a message for all parents or guardians of grade 7 students. Public Health nurses will be coming to your school on  November 3rd, 2017 to offer grade 7 students vaccines. All students are eligible to receive a vaccine against Tetanus, Diphtheria and Pertussis (whooping cough), and a vaccine against Human Papillomavirus (HPV). Your child should have received two information pamphlets.
 
Please complete the consent form, and return it to your child’s teacher. A signature is required for each vaccine on this form. This should be returned whether the student is being immunized or not.
 
Please contact the school as soon as possible if you did not receive immunization consent forms from your child. If you have any questions, please contact Public Health at
