Dear Parent/Guardian: 

Horizon Health Network Public Health will be offering the following two vaccines to all grade 9 students this year: 

1. Meningococcal vaccine (protection against invasive meningococcal disease caused by Neisseria meningitis type A, C, Y and W-135)
2. Varicella vaccine (protection against chickenpox disease). This is a booster dose to the Varicella vaccine received during childhood.
   
   Note: Your child does not need this vaccine if they have already received two doses or had chickenpox disease after one year of age that was confirmed by a blood test. 

The date for the clinic at Leo Hayes High School will be February 17, 2023.
