**ATTENTION GRADE 7 FAMILIES**

Grade 7 students will be bringing home information and consents for vaccinations that will be administered here at the school on October 20, 2017 and on April 20, 2018.  A signature is required for each vaccine on the Consent Form.  We would ask that you return the consent form as soon as possible but no later than OCTOBER 11, 2017.  ALL consents need to be returned whether you are accepting or refusing the vaccines.  If you have any further questions or concerns, please contact Public Health at 453-8252 or 453-5200.
